From Doody’s Book Review Service, September 21, 2007 **Description** This book provides a brief outline of the pathophysiology of chronic pain along with the diagnosis and treatment of common chronic painful conditions. It is outstanding in the clarity of the descriptions of various complicated pain phenomena. **Purpose** It is intended to provide a conceptual framework of understanding for anyone who deals with chronic pain patients. The book successfully achieves its objective of being a handy source of reference for physicians who want brief, up-to-date information on common pain conditions. **Audience** Although it is intended for any physicians who deal with chronic pain patients, including primary care physicians, it would be especially helpful for orthopedic surgeons and housestaff in any field who see patients on a regular basis where pain is a main complaint - those in neurology, rheumatology, and physical medicine and rehabilitation, for example. Pain is the most common complaint in the field of medicine and all physicians must have some knowledge of this area. **Features** The book starts with the anatomy, physiology, and molecular basis of chronic pain. Good chapters are devoted to myofascial pain, fibromyalgia, and tension headache. The chapter on trigger points is worthy of mention and the pharmacology of pain medications is nicely discusses in another chapter. Best of all is the chapter on the use of narcotics in chronic noncancer pain. The legal information and the guidelines, along with references, in this chapter are unprecedented. One chapter is dedicated to interventional pain techniques. The author rounds out the book with a chapter on psychological aspects of chronic pain and reviews interdisciplinary treatment of chronic pain in two other chapters. The book stands out in providing the most relevant information regardless of topic. This book emphasizes the medical aspects of chronic pain management, providing a nice balance between basic science and clinical information. The use of case reports helps in explaining the complete management of the patient. The book does not use fixed algorithms to "fix" pain patients; it is not for pain physicians who just want to learn injection techniques. The book lacks a dedicated evaluation of different injection therapies in the management of chronic pain. It does not discuss back pain enough, namely management of acute back pain, subacute back pain, chronic back pain, and the role of radiological workup at different stages, etc. Because back pain is so common, it needs as much attention as fibromyalgia or myofascial pain. The book has no tables, illustrations or diagrams, which certainly would have added value, especially when discussing pain anatomy and physiology. **Assessment** This is a valuable addition to the field of pain. It is very well written, discusses information clearly, and is well referenced while covering the most important topics in chronic pain. No other book of this size comes close to being as clinically useful as this one.
